What is Targeted Case Management?

At Kentucky United Methodist Children’s Homes (KyUMH), we walk alongside children, family, and individuals who have experienced trauma, abuse, and neglect in their lives. We are also working hard to provide services that would prevent those circumstances from happening.

Targeted Case Management (TCM) is a service we provide by ensuring people have access to the resources and support they need to be healthy and achieve overall wellness and stability. Many individuals and families are navigating mental health care, medical needs, school systems, or housing challenges all at once. These systems don’t often communicate well with one another. Many helpful resources may not be well known or easily accessible, which can be overwhelming.  

Through our work at Mending Point Behavioral Health, TCM is one way we help bring clarity and connection during these complicated situations. We make sure people have access to the right resources they need and that everything works together in a meaningful way.  

What does a Targeted Case Manager do?

When someone enters TCM, they are assigned a Targeted Case Manager. A Targeted Case Manager is someone who works alongside individuals and families to identify what they need, identify gaps in support, and connect them with the right resources.  

Through Mending Point, our case managers can help connect people to services across many areas of life, including:

  • Medical and behavioral health care
  • Substance use treatment and recovery supports
  • Housing, transportation and basic needs resources
  • Educational and vocational services
  • Social supports and community programs
  • Legal or court related resources

Rather than working in isolation, our Mending Point Targeted Case Managers help align efforts across providers.  

We also are rooted in a strengths-based approach. We believe every person has the capability and autonomy to reach their goals. Our role is to help people identify their strengths and how they can navigate their challenges.

What Targeted Case Management looks like day-to-day

TCM will look different case by case, as everyone’s goals and situations are different, but there are many ways our Targeted Case Managers at Mending Point will help you:

  • Making referrals and help clients access services
  • Support coordination between service providers, schools, and other agencies
  • Follow up to ensure services actually start, continue, and are effective in meeting the specific needs
  • Addressing obstacles such as transportation, paperwork, or scheduling
  • Tracking progress and continually reassessing needs, helping people adjust plans when circumstances change.

Our goal is to not only make connections, but to ultimately make sure those connections are effective and sustainable for each individual and family. That way, families can experience more stability and fewer disruptions in their day to day life.  

Why our work matters

At Mending Point, we know that systems and resources can be difficult to navigate. For individuals and families already facing stress and trying to manage difference circumstances alone, everyone deserves a fair chance and someone to walk alongside them when things get overwhelming.

TCM helps ease that burden. It creates a path forward by keeping services connected and focused on shared goals. In many cases, that support can help prevent more serious challenges down the road.
